A government buffeted by military and civil society (Comment)

The Congress' woes seem endless. No sooner had the party experienced a resounding rebuff in the recent assembly elections than it had to gird its loins to face a series of unprecedented challenges from the Chief of the Army Staff, Gen. V.K. Singh.

If the party and the government thought that the Supreme Court's obiter dicta on the age row involving the general had ended the matter, they were sadly mistaken because the army chief was not about to go quietly into the sunset. Evidently, he does not believe that the military, like children, should be seen and not heard.

So, he has turned on its head all the restraint of his predecessors, who maintained the British tradition of wearing a stiff upper lip. Whether or not Gen. Singh's garrulity is the result of the ubiquitous, 24x7 news channels, which weren't there during the time of the earlier army chiefs, the fact remains that the succession of "revelations" made by him - the offer of a bribe to him or about the obsolescence stalking the armed services in a letter to the prime minister - have shown up the government's seeming incapacity to govern - a charge which has haunted it ever since it let former telecom minister Andimuthu Raja run amok.

This impression may have gained ground because of the perception that neither Prime Minister Manmohan Singh nor Defence Minister A.K. Antony is the most assertive of men. If the age row had to be handled by the judiciary - just as it did with the telecom scandal - the bribery charge remained under wraps till Gen. Singh referred to it in a newspaper interview. A proactive government would have acted much earlier.

Similarly, it would not have been seen to have neglected defence preparedness despite the defence minister's warning in 2010 against the possibility of the country having to wage a two-front war against two nuclear-armed enemies.

The government as well as the general may now express outrage over the leakage of the letter. But where the ordinary citizen is concerned, the real outrage will be over the apparent erosion of the nation's military strength despite a 17 percent hike in the defence budget, especially in view of frequent reports about China's flaunting of muscles on the northeastern border.

The disclosures about a weakened army were all the more embarrassing when the BRICS Summit was being held in New Delhi and Chinese President Hu Jintao was in town. The pride of holding a major international event was marred by the disconcerting leaks even if the VIP guests were impressed by the vibrancy of the Indian media.

Needless to say, the revelations were a godsend for the opposition, especially the Bharatiya Janata Party (BJP), which has always projected itself as a champion of military strength and had unilaterally opted to showcase India's nuclear capability in 1998 as soon as it came to power at the centre. Now, all that it has said about the Congress' wimpishness has come true.

There is, therefore, little doubt that the Congress will be very much on the back foot in the coming weeks since nothing hurts a party more than the charge of having been negligent in matters of security. Being accused of corruption is bad enough. But to be remiss where the stiffening of the military muscle is concerned is an unpardonable offence in the eyes of the average person.

Not surprisingly, the government's inveterate adversary, civil society activists led by Anna Hazare, have lost no time in jumping into the fray by siding with the army chief. Earlier, they had decided to resume their anti-corruption agitation with the holding of a token fast in New Delhi and the launching of a jail bharo - fill the jails - campaign in August if no FIRs are filed against 14 union ministers who are corrupt in their view.

Evidently, Team Anna has not been deterred by their inability to organise a similar campaign after the Mumbai fiasco last December when Anna's rally failed to draw a crowd.

However, irrespective of whether the movement fizzles out this time as well, or whether there is a ceasefire between the government and General Singh following Antony's expression of confidence in the three service chiefs, the Congress and the government will continue to be buffeted by allegations of sleaze and slackness.

If the opposition hadn't been in such disarray - the BJP embroiled in factionalism, the regional parties cloistered in their local ghettos, the Left in a limbo - it would have been nearly impossible for the government to get off the mat. Even now, the Congress' seat share in parliament and voting percentage are bound to fall in the next general election unless there are visible signs of an increase in military preparedness and the passage of an effective Lokpal bill to check corruption.

Back in college, WC was required to read Whitaker Chambers? 1952 autobiography,?Witness. WC came to the book reluctantly. WC was a distant relative of Helen Gahagan Douglas, U.S. Representative from California, smeared by Richard Nixon as ?The Pink Lady.? Whitaker Chambers contributed to that effort, and afterwards to the Pumpkin Papers political theater that made Nixon (in)famous. But as a window into the mindset of the Cold War, Witness is pretty good, even if it is on The Heritage Foundation?s recommended reading list.

There?s one passage in the autobiography in which Whitaker is describing how he came to renounce Communism and the Soviets in particular. ?He tells this story of a European girl whose father had defected from the Communist ranks.

It was hard for her because, as an enlightened modern girl, she shared the Communist vision without being a Communist. But she loved her father and the irrationality of his defection embarrassed her. ?He was immensely pro-Soviet,? she said, ?and then ? you will laugh at me ? but you must not laugh at my father ? and then ? one night ? in Moscow ? he heard screams. That?s all. Simply one night he heard screams? . . . She did not know that she had swept away the logic of the mind, the logic of history, the logic of politics, the myth of the twentieth century, with five annihilating words: one night he heard screams.

The metaphor is vivid, forceful and unsettling, as any really powerful metaphor should be.

WC, many years ago, was staying with acquaintances in Anchorage, crashing at their place overnight on the way to a backpacking trip on the Kenai Peninsula. And that night he heard screams coming from his host and hostess?s bedroom. The next morning, WC?s hostess had obvious bruises, and refused to make eye contact with WC. That event, and the chance to really reflect on it over the course of the backpacking trip, marked the moment at which WC really understood marriage could be a prison, a trap and literal torture. Since then, WC has learned enough about the cycle of domestic violence to develop deep suspicion of those who uncritically defend marriage.

Which is why the National Organization for Marriage had earned WC?s distrust long before NOM was forced in litigation to disclose its National Strategy for Winning the Marriage Battle. WC wouldn?t want to have to decide what part of the Strategy is the most unsettling. Its defense of marriage at all costs is appalling, and its virulent anti-gay ambitions remind WC of the worst of the 1960?s Klu Klux Klan screeds.

For pure outrageousness, it?s hard to decide between NOM?s ?Catholic Clergy Project? and its ?Behind Enemy Lines? campaign. The former is described:

All clergy are key influencers on gay marriage, but Catholics are a key swing vote and Catholic clergy are notoriously difficult to personally reach. The Catholic Clergy Project aims to use NOM?s close relationships with Catholic bishops to equip, energize and moralize Catholic priests on the marriage issue.

We know a bit about Catholic clergy here in Alaska. The hundreds of children those Catholic clergy sexually abused know still more. If NOM wants to use them as role models here, then NOM is even sicker than WC had supposed. And, by the way, exactly what is it that those male, celibate priests ? the ones who don?t abuse children ? know about marriage?

The ?Behind Enemy Lines? project, in comparison, offers a $200,000 budget for ?a special effort to focus on the consequences of gay marriage for parental rights,? including a $120,000 item for ?Children of same-sex couples and their concerns ? an outreach coordinator to identify children of gay parents wiling to speak on camera.? Strategy, p. 24-25. A rebellious teenage kid never had more power.

WC invites his patient readers to decide which ?strategy? is more offensive.

Or, perhaps, the most offensive element is the refusal to recognize that abusive relationships and domestic violence in the relationship that they are defending is orders of magnitude worse than their perceived ?enemy,? gay marriage. NOM refuses to listen to ? refuses to admit the existence of ? the screams in the night.

One final note. WC is no fan of Governor Sean Parnell. But on this issue, the Governor ? or perhaps the First Lady pressuring the Governor ? deserves some modest praise. His campaign?to ?Choose Respect? is a good start at addressing Alaska?s shocking levels of domestic violence.

And they're seeking compensation from Bieber for his "reckless" behavior. </P><P> </P><P>The attorney tells the site that her clients received more than 1,000 phone calls from Bieber fans, at all hours of the day, and their patience "wears thin." So they're looking for restitution, which could come in any number of forms: An apology from JB himself, concert tickets for Dilcie's grandchildren, an endorsement for Kent's "upcoming online project," or &#8212; of course &#8212; cold, hard cash. </P><P> </P><P>Kent, who doesn't want his last name to be revealed, subsequently <a href="http://abcnews.go.com/GMA/video/justin-bieber-tweets-fake-phone-number-15892632" target="_blank">told ABC News</a> that he's had his number for 20 years, and he doesn't want to change it just because of Bieber. Of course, there <i>is</i> a question about whether Justin's initial tweets were even authentic, or if his account was somehow hacked. Coding for Hearing Tests: 92551 versus 92552

The difference between pure tone hearing tests and threshold hearing tests can confound coders. When do you use 92551 (Screening test, pure tone, air only) versus 92552 (Pure tone audiometry [threshold]; air only)? Tip: Keep in mind that one is a screening test and one is a diagnostic test.

Technical Details Point the Way

Knowing the technical differences and details of each hearing test will help coders choose the appropriate code.? The screening test is basically pass-fail, says Susan Beam, MA, CCC/A, an audiologist at Genesis Healthcare System in Zanesville, Ohio. During the test, a hearing screening device or audiometer produces a series of tones. The patient says whether he or she hears each tone. If the patient fails to hear one or more than one tone (depending on the office criteria for passing), he or she fails the test.

At this point, the family physician (FP) will either schedule a threshold test or refer the patient to an audiologist to determine the exact problem.

The threshold test is used to define the more specific diagnosis of the patients hearing problem, as opposed to the screening, which determines whether the patient has a hearing problem. Threshold testing identifies the softest level the patient can hear, Beam says. During the threshold test, the patient is presented with a series of tones at varying intensity (loudness). If the patient can hear the tone, the intensity is dropped in 10-decibel (dB) increments until he or she can no longer hear it. Next, the intensity is raised in 5-dB increments until the patient hears the tone again, then gradually dropped again, bracketing the threshold.

The physician records results of the threshold test in an audiogram, a graph charting the levels the patient can hear.

Coders can differentiate between the two tests by looking at the audiogram. The threshold will show a variation of different intensity levels recorded on the audiogram, Beam says. In contrast, results for the screening test will appear as just one level on the audiogram. Some physicians do not record screening results on the graph, but instead write in the chart screening.

Criteria for the Tests

Our doctor will conduct a hearing test on patients who have no signs and symptoms because he may want to fully examine a new patient, or he finds that an established patient has never had a hearing exam, saysPat Thompson, RHIT, coding analyst at Multicare HealthSystems in Tacoma, Wash. When the patient presents without a complaint, that test is a screening.

Medicare does not cover hearing screenings, but some commercial payers do. Coders should report 92551 for the screening with V72.1 (Special investigations and examinations; examination of ears and hearing). If the hearing screening is part of a routine physical, it can still be reported separately. For example, if a new patient has a preventive exam and the FP decides to conduct the hearing screening, code the appropriate preventive medicine code (99381-99387) and 92551. Link V70.0 (General medical examination; routine general medical examination at a health care facility) to the preventive medicine code and V72.1 to the 92551. Use V70.0 when performing the exam on adults. Use V20.2 (Routine infant or child health check) for screening tests on children.

Many practices code the definitive diagnosis for the hearing problem if the patient fails the hearing test. But despite the result of the test, the final diagnosis cannot be used with a screening exam. For example, an FP conducts a hearing screening on a 5-year-old new patient. The patient fails the test but has fluid in his ears causing conductive hearing loss. Report 92551 with V72.1. Use the diagnosis for the conductive hearing loss (e.g., 389.00) as the secondary code.

Code 92552 , on the other hand, is only for the diagnostic test. The patient has to present with signs and symptoms to use this code. If the patient fails a screening, the symptom is unspecified hearing loss [389.9], Beam says. If the physician is able to make a definitive diagnosis following the threshold test, code with the final diagnosis as primary and the symptoms as secondary. For example, a patient who works in a loud factory complains of hearing loss. The FP administers a threshold test and determines that the patient has high-frequency hearing loss. Bill 92552 with 388.12 (Noise-induced hearing loss) as the primary diagnosis and 389.9 as the secondary diagnosis.

Sometimes, the patient has been referred to his or her FP by an audiologist. Audiologists conduct group hearing screenings at schools or at work places that follow Occupational Safety and Health Administration (OSHA) rules. If a person fails the test during a group screening, the audiologist will send a report to the FP. The report informs the physician that the patient failed the test and that more extensive testing is needed. When the patient comes in for the threshold testing, report 92552 with the diagnosis for the signs and symptoms, unless a more definitive diagnosis is found. For example, a child is seen for an office visit with a note from the school that he failed two hearing screenings. The physician examines the patients ears, performs a threshold test and determines the child has otitis media. Report 92522 with the diagnosis code for otitis media (382.9, Unspecified otitis media).

Although FPs usually test both ears, sometimes they will test only one when the patient reports hearing loss in just one. Because both 92551 and 92552 refer to testing both ears, coders need to append modifier -52 (Reduced services) to the hearing test code when only one ear is tested. Coding experts suggest reducing the fee when appending modifier -52. Also, practices should submit a KISS (keep it short and simple) letter when they use this modifier explaining why the services were reduced."

Susan Blumenthal, M.D.: World Tuberculosis Day 2012: Stopping Tuberculosis in Our Lifetimes

This Saturday, March 24 is World Tuberculosis Day, marking the 130th anniversary of Dr. Robert Koch's discovery of Mycobacterium tuberculosis, the microbe that causes tuberculosis. This year, the Stop TB Partnership, an international collaborative of governmental and non-governmental organizations, the private sector, and others dedicated to fighting tuberculosis, has adopted "Stop TB in My Lifetime" as its theme for World Tuberculosis Day. Yet, nearly a century and a half after Koch's discovery, stopping tuberculosis in our lifetimes is a formidable challenge. A $1.7 billion dollar shortfall for the Global Fund to Fight AIDS, TB and Malaria over the next five years means that 3.4 million patients may go untreated. This could reverse the important gains that have been made over the past years in the fight against this disease. The Global Fund provides 80 percent of external resources for TB eradication and treatment. Since this public/private partnership was established in 2002, the organization has helped detect and treat 8.6 million cases of TB [1].

TB is one of the most widespread infectious illnesses in the world -- with an estimated one-third of people globally infected with the bacteria that causes TB according to the World Health Organization (WHO). In 2010, the World Health Organization (WHO) reported that there were an alarming 8.8 million new cases of active tuberculosis and 1.4 million deaths caused by this disease, making it the eighth leading cause of death globally [2,3]. However, these numbers represent a decline in new cases since 2005. With 95 percent of TB deaths occurring in developing countries, tuberculosis is a public health problem that disproportionately affects the poor and young adults in their most productive years [4]. Furthermore, there were 9.7 million orphaned children as a result of their parents' death from TB.

Significantly fewer people die of tuberculosis in developed countries, but the disease nonetheless remains a health threat in the United States, with more than 10,521 cases reported in 2011 [5,1]. TB is a public health tragedy, given that it is both preventable and curable. However, this week there was some good news: TB infections in America have dropped to record lows, falling 6.4 percent in 2011, but still missing a national target for eliminating the disease. TB rates are 12 times higher among people living in the U.S. who were born outside the country. Compared with whites, TB rates were seven times higher for Hispanics, eight times higher for blacks, and 25 times higher for Asians in America last year. Four states -- California, Texas, New York, and Florida -- account for nearly half of all TB cases in the United States. According to a recent CDC report, unless circumstances change significantly, the U.S. will not be able to eradicate TB until 2100.

The symptoms of tuberculosis include coughing, chest pain, fatigue, fever, weight loss, chills, night sweats, and loss of appetite. While TB most often affects the lungs, it can also affect the brain, kidneys, or spine. The disease can spread when an individual with active tuberculosis -- a disease state characterized by actively dividing bacteria -- coughs, sneezes, talks, or spits, releasing droplets containing M. tuberculosis into the air. If other people inhale these droplets, they may acquire the disease. Without treatment, individuals with active TB will infect an average of 10 to 15 people each year, and can ultimately die from the disease. Individuals with latent tuberculosis -- characterized by non-dividing bacteria -- are symptomless and unable to infect others, but they remain at risk of developing active disease if they are not treated. It is estimated that one-third of the world's population currently has latent tuberculosis [6].

Significant progress has been made in the fight against TB in recent decades. Since 1990, there has been a 40 percent decline in the global death rate for this disease, and the world was on track until the recent funding shortfalls to achieve the Millennium Development Goal 6 target that aims to halt and reverse the tuberculosis incidence rate by 2015 [3,4].

One major advance has been the implementation of Directly Observed Therapy Short-Course (DOTS), a multi-pronged tuberculosis control strategy best known for its use of health care workers to supervise the administration of TB therapy. This approach has dramatically improved treatment rates and patient survival. DOTS has five key components: 1) sustained political and financial commitment; 2) diagnosis by quality-ensured sputum-smear microscopy; 3) standardized short-course anti TB treatment given under direct and supportive observation; 4) a regular, uninterrupted supply of high quality anti-TB drugs; and 5) standardized treatment and reporting. Globally, by 2007, 99 percent of all cases of TB reported to WHO were being treated in DOTS programs. In 2009, DOTS had a treatment success rate of 87 percent -- its highest level to date. Since 1995, 46 million people have been successfully treated and up to 6.8 million lives have been saved through DOTS and the Stop TB Strategy. And this month, the World Health Organization (WHO) announced that its integrated prevention, diagnosis, and treatment efforts for tuberculosis and the human immunodeficiency virus (HIV) prevented an estimated 910,000 deaths in six years [7].

While these efforts have made a lifesaving difference for many, much more remains to be done. Here is why:

  • Tuberculosis prevention is limited by the lack of an effective vaccine. The existing Bacillus Calmette-Gu?rin (BCG) vaccine has limited success in preventing tuberculosis infection. When successful, protection is estimated to last around 15 years. Furthermore, the vaccine has been shown to cause potentially fatal infection when given to HIV-positive adults or to children with weak immune systems [9].
  • Many resource-poor countries lack the necessary infrastructure, equipment, and trained personnel needed to accurately diagnose tuberculosis and differentiate between drug-resistant strains. As a result, many infected people remain undiagnosed or receive non-effective treatments. The WHO's recent endorsement of the Xpert MTB/RIF, a diagnostic test that is capable of detecting M. tuberculosis and resistance to the drug rifampicin in two hours, represents a major step forward in reducing the time required for diagnosis and detection of multi-drug resistant (MDR) tuberculosis (the type that is resistant to the two most powerful first line TB drugs). However, there is still a need for additional rapid diagnostic tests that are both accurate and affordable for use in the developing world.
  • Existing treatment regimens require patients with active tuberculosis to take multiple drugs for months at a time, making treatment adherence challenging. This, along with the prescribing of incorrect drugs, has contributed to the emergence and spread of MDR and extensively drug-resistant (XDR) tuberculosis. In 2010, there was an estimated prevalence of 650,000 cases of multidrug-resistant TB (MDR-TB) and in 2008, 150,000 deaths annually. Because these types of tuberculosis are very difficult and costly to treat, they provide significant challenges to TB control efforts.
  • TB control efforts are also complicated by the co-existence of tuberculosis and HIV/AIDS. Tuberculosis is the leading cause of death for people with HIV/AIDS worldwide. Because people with HIV/AIDS have weakened immune systems, they have a 20-37 times greater risk of developing active tuberculosis than people without HIV/AIDS [8]. Yet rates of TB testing among HIV patients and provision of ARV therapy to co-infected individuals remains low.
  • Current projected shortfalls in funding are jeopardizing the progress that has been made in tuberculosis care and control. Worldwide, the share of domestic funding for TB provided by affected countries rose to 86 percent. But most low-income countries still depend on external funding with The Global Fund to Fight AIDS, Tuberculosis and Malaria -- an international public-private funding institution to which the United States is the largest contributor -- providing 82 percent of international TB support in 2012. Other important sources of funding include President Obama's Global Health Initiative (TB is one of GHI's six focus areas), PEPFAR, and the Gates Foundation [3, 9]. In March 2010, in response to Congressional reporting requirements to develop a coordinated approach to global TB, several U.S. government agencies released a joint TB strategy to accelerate the diagnosis, treatment, and prevention of TB; scale up prevention and treatment of MDR-TB; expand coverage of interventions for TB-HIV co-infections in coordination with PEPFAR programs; and strengthen national health systems. The United States currently provides bilateral TB assistance to 40 countries. Alarmingly, due to the current economic crisis, U.S. funding for PEPFAR may be cut. Furthermore, the Global Fund to Fight AIDS, Tuberculosis and Malaria has announced that due to limited resources this year, it would postpone all new program grants until at least 2014. Overall funding for tuberculosis is expected to total $4.4 billion in 2012, which falls $1.7 billion short of projected needs. At $0.6 billion, funding for MDR tuberculosis faces a resource gap of $200 million. This lack of resources jeopardizes the gains that have made against this disease in recent years [1].

Addressing each of these challenges will be essential to eliminating tuberculosis in the years ahead. Many of the tools currently used for TB prevention, diagnosis, and treatment are antiquated and have limited success rates. Given the toll that TB takes on the world's population, an effective vaccine, rapid diagnostic tests, shortened, effective treatment regimens, and more research on the illness are required. That's why increased funding is urgently needed to develop 21st-century approaches. In order to ensure that new tools are developed and today's treatments are provided, governments must prioritize tuberculosis as a public health priority and close the projected gaps in domestic and international funding. In areas with a high burden of HIV/AIDS, scaling up joint HIV/TB prevention, diagnosis, and treatment activities will play a critical role in reducing tuberculosis and HIV-related deaths.

Nearly one and a half centuries have passed since Koch's discovery of the causative agent of tuberculosis; significant progress has been made with declining death rates globally but the disease nonetheless remains a leading killer in the developing world and a public health threat in the United States as well. As UN Secretary-General Ban Ki-moon recently underscored, intensified global solidarity is needed. "Countries must implement policies that not only raise awareness about the issue but provide accessible health care to their citizens." For those affected by the disease, stopping tuberculosis cannot wait a lifetime.

Who is Jim Yong Kim, nominee for World Bank president?

The selection of Jim Yong Kim took many by surprise since he is not well known in Washington circles and wasn?t an expected candidate for the World Bank position.

President Obama has nominated Jim Yong Kim, a global health policy expert and the president of Dartmouth College, to run the World Bank.

The selection took many by surprise since Mr. Kim is not well known in Washington circles and wasn?t an expected candidate for the position. At least one other person had openly campaigned for the position, which was a break in protocol.

Before taking the reins at Dartmouth, Kim was active in health issues in developing countries for much of his career. Between 2004 and 2006, Kim was director of the World Health Organization?s HIV/AIDS Department. He was a co-founder of Partners in Health, which provides health care to the poor, and has worked on international tuberculosis policy.

Since the White House traditionally selects the president of the World Bank, Kim is now expected to take the helm. The current president, Robert Zoellick, will step down on June 30 after a five-year term.

?Jim has spent more than two decades working to improve conditions in developing countries around the world,? said Mr. Obama in a statement. ?The World Bank is one of the most powerful tools we have to reduce poverty and raise standards of living around the globe, and Jim?s personal experience and years of service make him an ideal candidate for this job.?

The World Bank aims to reduce poverty and support development by giving financial and technical assistance to developing countries. Criticism of the World Bank ranges from disagreements over what kind of assistance is best to concerns about corruption to dissatisfaction with the United States having so much say in choosing the World Bank leader.

After Mr. Zoellick announced last month he was stepping down, Jeffrey Sachs, director of The Earth Institute at Columbia University, raised eyebrows by launching a campaign to lead the World Bank.

But some saw that as a good thing, raising the bar for who could be nominated. Mr. Sachs is also an economist with extensive experience in economic development as well as a health expert who, like Kim, has worked to fight diseases such as HIV/AIDS and tuberculosis. Sachs advocated reform for the World Bank.

?Once Sachs [came forward], it was clear it would be very difficult for the Obama administration to follow past practice and simply choose, again, a political insider or a banker,? says Mark Weisbrot, co-director of the left-leaning Center for Economic and Policy Research in Washington.

Despite not getting the nomination, Sachs expressed his support for Kim via Twitter.

?Jim Kim is a superb nominee for WB. I support him 100%. I thank all who supported me and know they'll be very pleased with today's news,? he tweeted.

Several African countries endorsed Nigerian Finance Minister Ngozi Okonjo-Iweala, which was seen as a way to break the US hold on deciding World Bank presidents. Jos? Antonio Ocampo, Colombia?s former Finance minister, was also put forward.
